Although "scapholunate dissociation" is a descriptive term, it is used synonymously with its primary pathophysiologic correlate, scapholunate interosseous ligament (SLIL) injury. Although injury to additional ligaments is present in scapholunate dissociation, the SLIL is the major target of surgical reconstruction.

The various stages of scapholunate dissociation serve as key factors influencing management and treatment. In the setting of occult instability, the treatment is conservative with casting, medication (NSAIDs), physical therapy, and sometimes arthroscopic debridement. If you had a recent scapholunate ligament injury, the early goal of treatment will be pain control. This may include wearing a wrist splint or cast, decreasing activity and resting the wrist. This may take 6 or more weeks to feel better. Sometimes hand therapy can be helpful if the wrist is stiff after wearing a splint or cast. Building a better ligament Management of scapholunate instability is both complex and controversial.

September 8th, 2008. An acute complete tear of the ligament between the scaphoid and lunate bones commonly results from a forceful hyperextension injury to the wrist---for example after a fall onto an outstretched hand or a car accident. Such a complete tear allows the scaphoid bone to flex and the lunate to extend---a type of carpal instability referred to as a DISI deformity. 1.

Although good to excellent results have been reported using a variety of surgical techniques, they have often not been confirmed by other authors. Limited wrist mobility and the likelihood that degenerative changes will progress appear to be inherent in most approaches. 2016-12-01 scapholunate dissociation were review-ed and current methods used for clinical examination and ima~ging techniques were investigated. The literature con-cerning the various treatment modalities was then analyzed. An evidence-based assessment was conducted to evaluate the outcomes of different treatment methods.
